Qualifications

• MSW degree from a CSWE-accredited school.
• Minimally licensed in the state where OSH Center is located.
• Fluency in Spanish, Polish, Russian, or other languages spoken by communities we serve.
• Proficient with computer systems (Microsoft, GSuite, EMR).
• US work authorization.

Responsibilities

• Provide assessments, telephonic check-ins, and in-person visits for high-need patients.
• Collaborate with patients and support persons to set goals and execute care plans.
• Refer to community resources to address health and psychosocial needs.
• Navigate insurance benefits with the Patient Relations Manager to maximize benefits.
• Deliver longitudinal and episodic care coordination to empower patients to manage their health.
