Job summary

Oak Street Health is seeking a Medical Social Worker to join its care team. The MSW will conduct comprehensive assessments, provide advocacy, and coordinate care for high-risk patients and their caregivers to reduce hospitalizations and improve health outcomes.

In collaboration with interdisciplinary teams, you will identify goals, navigate insurance benefits, and connect patients with community resources to support ongoing wellness and quality of life.

Qualifications

• Master of Social Work degree required from a CSWE-accredited school.
• Minimally licensed in the state where OSH Center is located.
• Fluency in Spanish, Polish, Russian, or other languages spoken in the communities served.
• Proficient computer skills (Microsoft Office, GSuite, EMR).

Responsibilities

• Provide comprehensive assessments, telephonic check-ins, and in-person visits for high-need patients.
• Collaborate with patients and their support network to set goals and drive care plan execution.
• Refer to community resources to meet healthcare and psychosocial needs.
• Navigate insurance benefits with the PRM to maximize plan benefits.
• Deliver longitudinal and episodic care coordination to empower patients to manage health conditions.
• Educate patients on care options and provide goals of care counseling and advocacy.
• Offer psychoeducation to patients and families on life stage and disease-specific issues.
