Job summary

Oak Street Health is seeking a Medical Social Worker to join our care teams in New Haven, CT. The MSW will conduct assessments, coordinate care, and advocate for patients with complex psychosocial needs to improve health outcomes and reduce hospitalizations.

Required: CSWE-accredited MSW, licensure in the state, US work authorization, and fluency in Spanish, Polish, or Russian. Proficiency with EMR, Microsoft and GSuite is expected.

Qualifications

• MSW degree from CSWE-accredited program.
• Licensed in the state where OSH Center is located (minimum).
• Fluency in Spanish, Polish, Russian, or other community languages as needed.
• Proficient computer skills (Microsoft, GSuite, EMR).
• US work authorization.

Responsibilities

• Proactively assess high-need patients and conduct telephonic check-ins and in-person visits.
• Collaborate with patients and support persons to set goals and execute care plans.
• Refer to community resources to meet healthcare and psychosocial needs.
• Navigate insurance benefits with the PRM to maximize coverage.
• Provide longitudinal and episodic care coordination to empower self-management.
