Self-Help For The Elderly in San Francisco, CA is seeking a Medical Social Worker for our Home Care & Hospice teams. The role involves assessing psychosocial needs, developing care plans, and coordinating with an interdisciplinary team to support patients and families in their home and hospice settings.

The position requires an MSW, valid social work licensure, bilingual proficiency in English and Cantonese, and strong documentation skills.

Qualificações

• Licensed as a clinical social worker by the Board of Behavioral Sciences.
• MSW from CSWE-accredited program with one year of health care social work experience.
• Cal. Code Regs references and regulatory awareness.
• One year of health care, home health care, or hospice experience preferred.
• Bilingual in English and Cantonese; Mandarin & Toishanese are a plus.

Responsabilidades

• Assess psychosocial and emotional needs of patients and families.
• Develop plan of care to support coping with illness and practical arrangements.
• Maintain clinical records and documentation up to standards.
• Provide information and referrals to patients, families, and community agencies.
• Collaborate with interdisciplinary team and participate in case conferences.
• Assist with funeral planning, financial and health care decisions as needed.
• Travel as required; maintain CPR certification and health screenings.
