Job summary

Qualifications

• Masters degree in Behavioral Science or Social Work.
• Licensure in Washington as LISW, LMHC, LSW Advanced, or LMFT.
• Excellent written and verbal communication skills.
• Demonstrated success as part of a multi-disciplinary team and commitment to diversity.
• Minimum 1 year social work experience.
• Two years post-masters experience preferred.

Responsibilities

• Provide social work interventions to patients of all ages and their families.
• Include psychosocial assessment, brief individual or family therapy, advocacy, linkage, referral.
• Coordinate with multidisciplinary team and conduct family conferences.
• Assess patient and family needs for care and communicate with health care providers.
• Modify plan of care based on patient and care system response.
