Job summary

Sierra Health Care is seeking a Licensed Clinical Social Worker to join our home health and hospice team in New Mexico. You will work with patients and families in their homes, providing counseling, psychosocial support, and end-of-life planning as part of an interdisciplinary team.

The role emphasizes clinical social work, compassionate caregiving, and strong documentation within HIPAA-compliant, regulated home health settings.

Qualifications

• Master's degree in Social Work (CSWE-accredited program)
• Current, unencumbered LCSW license in New Mexico
• Two or more years of clinical social work experience preferred
• Competency in behavioral health, family dynamics, and counseling techniques
• Strong documentation and communication skills
• EMR proficiency and ability to learn hospice and home health documentation systems
• Reliable transportation, valid driver’s license, and auto insurance for home visits

Responsibilities

• Conduct mental health and behavioral health assessments under a physician's plan of care
• Provide counseling, psychotherapy, and psychosocial support to patients and families
• Support grief counseling, end-of-life decision-making, and advanced care planning
• Contribute to diagnosis and care planning for patients with complex emotional and social needs
• Collaborate with the interdisciplinary team to coordinate care across clinical, emotional, and social domains

Key Responsibilities

• Conduct mental health and behavioral health assessments under a physician's plan of care
• Provide counseling, psychotherapy, and psychosocial support to patients and families
• Support grief counseling, end-of-life decision-making, and advanced care planning
• Contribute to diagnosis and care planning for patients with complex emotional and social needs
• Collaborate with the interdisciplinary team to coordinate care across clinical, emotional, and social domains

Other Duties:

• Provide care and services in accordance with infection control, safety, and risk prevention protocols
• Accurately document clinical interventions, care coordination, and communication in a timely manner
• Participate in interdisciplinary meetings, case reviews, and continuing education opportunities
• Maintain compliance with HIPAA and all company privacy and confidentiality standards
• Represent the organization positively and professionally in patients' homes and community settings
• Work independently with minimal daily oversight and demonstrate initiative in completing responsibilities
• Provide courteous, timely, and professional customer service to patients, families, and partners
• Exhibit adaptability to changing responsibilities and flexibility in completing assignments
• Demonstrate consistent and effective use of organizational technology systems and tools required for the role, including communication, documentation, productivity, and data management platforms
• Adhere to all organizational policies, including job descriptions, mission, and the Employee Handbook
• Communicate and interact professionally and respectfully with others to support team goals
• Perform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the company

Requirements
