Klarus Home Health / Jet Health Inc. is seeking a Licensed Medical Social Worker to provide coordinated in-home psychosocial care for patients of all ages in Albuquerque, NM and surrounding areas.

The LMSW will assess needs, develop care plans with families, and educate patients and caregivers while maintaining clear documentation and collaborating with the health care team. Active licensure and CPR certification are required.

Qualifications

• Ongoing clinical assessments of home health patients to identify psychosocial needs.
• Counseling and education to patients, clients, and caregivers as ordered by physicians.
• Collaborative care planning and coordination with patient, client, and caregiver(s).
• Maintain accurate, timely documentation of assessments and communications with providers.

Responsibilities

Klarus Home Health / Jet Health Inc. is currently seeking a Licensed Medical Social Worker (LMSW) to join our team to serve our patients currently enrolled in our home health services in the Albuquerque, NM region and surrounding areas.

The successful candidate will provide coordinated care in the home to patients of all age groups, performing psychosocial assessments, analysis, counseling, and referrals to meet the needs of the patient, client, and family. Requirements

• Conduct ongoing clinical assessments of home health patients to identify psychosocial, financial, environmental, and community resource needs.
• Develop and evaluate the plan of care in collaboration with the patient, client, representative (if any), and caregiver(s).
• Provide counseling and education to patients, clients, and caregivers as ordered by the physician and indicated in the plan of care.
• Maintain accurate and timely documentation of clinical assessments, interventions, and communication with other healthcare practitioners.
• Utilize effective interpersonal relations and communication skills to facilitate positive outcomes for patients and their families.
• Collaborate with the interdisciplinary team to promote coordination of patient care and ensure comprehensive support.
• Participate in the Agency's Quality Assessment and Performance Improvement (QAPI) program and in-service training.
• Stay updated on public and private eligibility standards and requirements, maintaining professional growth and competency.
• Promote the Agency's philosophy and administrative policies to ensure the delivery of quality care.

Requirements

• Master's or Doctoral Degree from an accredited school of Social Work.
• Active social work licensure (LMSW, LICSW, or LCSW) in the state of practice.
• Minimum of one year experience as a Master of Social Work (MSW) in a healthcare setting.
• Working knowledge of community resources and good interpersonal skills.
• Proof of current CPR certification.
• Reliable transportation with a valid driver's license and auto liability insurance.
