Job summary

Corus Health is seeking a Medical Social Worker to join our hospice team. In this field role, you will support patients and families at home, helping them understand options, access resources, and navigate care with compassion.

You will collaborate with the interdisciplinary team, develop individualized care plans, document assessments, and advocate for psychosocial needs within Medicare CoPs and state regulations. A CSWE-accredited MSW and NM licensure are required.

Qualifications

• Master's degree in Social Work from a CSWE-accredited program.
• LMSW or LCSW license as required by the state of New Mexico.
• At least one year of social work experience in a healthcare setting.
• Knowledge of community resources and psychosocial care planning.
• Strong communication, assessment, and advocacy skills.
• Comfort with EMR systems and basic computer skills.
• A valid driver's license, reliable transportation, and auto insurance for home visits.
• The independence and initiative to do this work well in the field, with a team behind you.

Responsibilities
