Job summary

Corus Health seeks a Medical Social Worker for hospice care in the Albuquerque area. You will support patients and families, outline psychosocial options, and coordinate services with an interdisciplinary team.

The role emphasizes compassion, calm, and clear communication in the field and home settings, with a mission-driven approach to care.

Qualifications

• Master's degree in Social Work (CSWE-accredited)
• LMSW or LCSW license as required by New Mexico
• Minimum 1 year of healthcare social work experience
• Knowledge of community resources and psychosocial care planning
• Strong communication, assessment, and advocacy skills
• Comfort with EMR systems and basic computer skills
• Valid driver's license, reliable transportation and auto insurance for home visits
• Ability to work independently in the field with a supportive team

Responsibilities

• Provide social work services to hospice patients and families under a physician's plan of care
• Help the interdisciplinary team understand psychosocial factors shaping care
• Connect patients and families with community resources, financial support, and social services
• Develop and carry out individualized care plans
• Contribute to planning for safe, smooth transitions
• Partner with community organizations and make referrals when helpful
• Join patient care conferences and consult with teammates
• Document assessments and interventions clearly and on time per Medicare CoPs and state regs

Key Responsibilities

• Provide social work services to hospice patients and families under a physician's plan of care
• Help the interdisciplinary team understand the social, emotional, and environmental factors shaping each patient's care
• Connect patients and families with community resources, financial support, and social services
• Help develop and carry out individualized care plans
• Contribute to planning that supports safe, smooth transitions
• Partner with community organizations and make referrals when they will help
• Join patient care conferences and consult with your teammates
• Document your assessments and interventions clearly and on time, in line with Medicare CoPs, state regulations, and professional social work standards

What We're Looking For

• A Master's degree in Social Work from a CSWE-accredited program
• LMSW or LCSW license as required by the state of New Mexico
• At least one year of social work experience in a healthcare setting
• Knowledge of community resources and psychosocial care planning
• Strong communication, assessment, and advocacy skills
• Comfort with EMR systems and basic computer skills
• A valid driver's license, reliable transportation, and auto insurance for home visits
• The independence and initiative to do this work well in the field, with a team behind you
