Job summary

Deaconess Home Health in Evansville, Indiana is seeking a PRN Medical Social Worker to join our team. You will help patients and families navigate social, emotional and economic barriers, coordinate care plans with physicians, and connect clients with community resources.

Requirements include a Master’s degree in Social Work, Indiana licensure, CPR/BLS certification, and at least one year of health-care social work experience.

Qualifications

• Master’s degree in Social Work from an accredited program.
• Current state license to practice social work in Indiana.
• CPR/BLS certification and compliance with state requirements.
• Minimum one year of health-care social work experience.

Responsibilities

• Establish a care plan aligned with the physician’s treatment plan.
• Provide crisis intervention as needed.
• Help caregivers understand social, emotional and economic factors affecting care.
• Educate patients and caregivers about care and resources.
• Refer to community resources and public assistance when needed.
• Collaborate with caregivers to resolve barriers to care.
• Complete and submit documentation accurately and timely.

NO After Hour On Call requirements

Responsibilities

• Establishing a care plan in accordance with the physician’s plan of treatment.
• Providing crisis intervention as required.
• Assisting the physician and other caregivers in understanding the significant social, emotional and economic factors influencing the patient’s ability to improve medically.
• Providing patient care education to patients and caregivers.
• Assisting in making appropriate community referrals to gain access to additional resources as needed (i.e. applications for public assistance, housing problems, additional health care resources, transportation)
• Working with the patient’s caregivers to resolve problems interfering with an optimum caring relationship between the patient and caregiver.
• Completing and submitting all required documentation in an accurate and timely manner.

• Mileage Reimbursement (58.5 Cents per mile)
• No After hour on call requirements

Job Requirements

• Master’s degree from an accredited school approved by the Council on Social Work Education. In Indiana, must also be certified under IC25-23.6-4-3.
• Currently licensed to practice in the state serviced.
• One year social work experience in a health care setting.
• Current CPR/BLS certification through American Heart Association or American Red Cross and compliance with state requirements

Pay Range

$26.70 - $39.71 /hour
