Job summary

CareAparent is hiring a full-time Medical Records Specialist in Woodbury, MN. You will manage patient data, maintain EMR workflows, and ensure accurate records for multiple services in a privacy‑conscious environment.

You will collaborate with clinical, operational, and compliance leaders to implement processes that protect PHI and improve workflows. Prior experience with EMR systems and HIPAA is valued.

Qualifications

  • Associate’s or advanced degree required.
  • 1-3 years of healthcare experience.
  • Minimum 1 year of EMR experience.
  • At least 1 year in medical records/health information.
  • CEHRS or HIMSS certification preferred.
  • Knowledge of medical terminology and HIPAA.

Responsibilities

  • Coordinate with management to collect and resolve client/patient data.
  • Collaborate with clinical, operations, and compliance teams on workflows for EMR platforms.
  • Maintain active and discharge client rosters for all services.
  • Process requests for medical records from clients, affiliates, and payors.
  • Process Release of Information requests for records.
  • Develop and implement standards for medical records processing.
  • Report breaches or noncompliance to protect PHI.
  • Support quality improvement projects across CareAparent.
  • Monitor clinical outcomes and data in the EMR; assist with discharges.
  • Handle faxing, filing and follow-up on physician orders and authorizations.
  • Ensure client records comply with state and federal regulations.
  • Adhere to HIPAA and health information privacy rules.
  • Provide backup support to Home Health and Intake teams.

We are currently seeking a full-time Medical Records Specialist to add to our office team in Woodbury, MN!

At CareAparent you will collaborate with other passionate individuals focused on giving the best care possible for our clients. This position reports directly to the SVP of Operations. This is the first role of its kind and CareAparent and will be instrumental in building the foundation of CareAparent’s processes and procedures for handling medical records in both their physical and electronic formats. This role will partner with clinical, operational, and compliance leaders to streamline workflows and serve our clients and partners in a secure, efficient, and accurate fashion.

Hours, Location & Wage:
  • 8:00 am – 5:00 pm, Monday-Friday (40 hours/week)
  • In-office position in Woodbury, MN
  • Wage: $24.00 – $28.00/hour

Position Responsibilities:
  • Consult management stakeholders to identify, discuss and solve client/patient data.
  • Work with clinical management to create, enhance, and train on new or modified workflows as it relates to WellSky Home Health and Personal Care (EMR’s).
  • Maintain CareAparent’s official active and discharge client rosters for all services.
  • Retrieve, collect and prioritize all requests for medical records made through the organization, clients, affiliates, and payors.
  • Process Release of Information requests for medical records.
  • Review, develop and implement standards and procedures for processing medical records requests.
  • Report breaches, instances of noncompliance, client complaints, problems, or similar instances to supervisor to protect client PHI.
  • Participate in quality improvement projects throughout CareAparent.
  • Review, develop and implement clinical outcomes and performance, referral, and payor data in EMR.
  • Monitor and track hospitalization updates and assist with discharges.
  • Fax, file, and follow-up on physician orders and authorizations.
  • Maintain and manage client records in accordance with state and federal regulations.
  • Adhere to HIPAA and other healthcare regulations governing CareAparent.
  • Provide support to Home Health and Intake teams as assigned.
Qualifications:
  • Associate’s or advanced degree required
  • 1-3 years of healthcare experience
  • A minimum of 1 year of experience using an Electronic Medical Record (EMR) platform
  • At least 1 year of medical records or health information experience
  • Home healthcare experience preferred
  • A Certified Electronic Health Records Specialist (CEHRS) or HIMSS certification preferred
  • Knowledge of medical terminology and HIPAA Compliance
