Job summary

Trinity Health PACE is seeking a Medical Records Specialist to manage electronic and hard-copy records for the PACE organization. This full-time, onsite role requires attention to confidentiality, strong communication, and teamwork within a patient-centered environment.

You will handle requests, coordinate with coders, maintain EMR records, and support training across local PACE clinics. Local travel may be required.

Qualifications

• High School Diploma or equivalent is required.
• At least 1 year of medical records experience.
• Strong written and verbal communication skills.
• Proficiency with Microsoft Office and email/calendaring.
• Position may require up to 10% local travel.

Responsibilities

• Provide administrative support to program committees and ensure staff understand and respect participant rights.
• Participate in surveys and assist in creating corrective action plans for departmental issues.
• Ensure confidentiality of all medical records.
• Fulfill medical record requests from Enrollment, PACE providers, nurses, and finance staff.
• Coordinate with PACE coders to obtain missing documentation.
• Maintain an updated list of fax numbers for providers, hospitals, and facilities.
• Log all incoming and outgoing medical record requests.
• Request records securely via fax with necessary authorizations.
• Upload and annotate received records in the EMR for easy access.
• Notify Medical Director and Providers when records are received.
• Respond to internal requests for EMR records.
• Track all records sent out, including dates and contacts.
• Support additional projects as needed.
• Maintain an up-to-date contact list of healthcare providers and assist with admin tasks.

Position Purpose

Minimum Qualifications

• High School Diploma/equivalent required.
• One (1) year of medical records experience required.
• Superior written/verbal communication skills.
• Must be customer focused and flexibility to adapt to ongoing change.
• Proficiency with Microsoft Office, electronic mail and calendaring systems.
• Ability and skills to plan, organize, develop, implement and interpret programs, goals, objectives, policies and procedures of the organization in line with our mission, vision, and philosophy.
• Position may require 10% local travel.
