Optimal Home Care is seeking a Medical Records Coordinator to support patient records, scheduling accuracy, and quality improvement in a Colorado home health setting.

Qualifications

• High School Diploma or GED required; Bachelor’s degree preferred.
• Experience with computer and general office duties in a home health setting is desirable.
• Proficient in Excel, Word, Outlook and Adobe.

Responsibilities

• Provide team approach to delivering quality service to patients and staff.
• Read and respond professionally to emails and phone calls promptly.
• Monitor faxes, perform quality inspection and route documents to the proper department.
• Organize and file SOC documentation in patient charts.
• Upload physician orders timely to assist scheduling accuracy.
• Ensure MD orders and lab results are uploaded correctly.
• Handle wound care photos and upload them accurately.
• Run reports to verify data accuracy and identify process improvements.
• Distribute medication profiles by mail within 14 days of SOC.
• Compile and handle medical record requests per HIPAA regulations.
