Job summary

CVS Health is seeking a full-time Medical Records Auditor to perform audit and abstraction of medical records, identify accurate ICD codes, and ensure documentation supports CMS risk adjustment processes. You will work to maintain compliance with state and federal regulations and internal policies.

Responsibilities include supporting coding judgments, ensuring accuracy of diagnoses, and conducting self-audits while meeting project deadlines.

Qualifications

• Minimum 1 year of related experience in medical record documentation review, diagnosis coding, and/or auditing.
• CPC (Certified Professional Coder) or CCS-P (Certified Coding Specialist-Physician) required.
• Experience with ICD codes required.

Responsibilities

• Audit and abstraction of medical records to identify and submit ICD codes.
• Support coding judgment using industry standard evidence and tools.
• Ensure codes are accurate and supported by clinical documentation.
• Identify active vs historical conditions and ensure medical necessity.
