Job summary

CVS Health is seeking a Medical Records Auditor to perform audit and abstraction of medical records for ICD coding submitted to CMS for risk adjustment. You will support coding judgment, ensure accuracy, and maintain compliance with CMS, OIG, and internal policies.

The role requires 1 year of related experience, CPC or CCS-P certification, and proficiency with MS Office and coding applications. A strong attention to detail and ability to meet deadlines is essential.

Qualifications

• 1 year of experience in medical record documentation review, diagnosis coding, and/or auditing.
• CPC (Certified Professional Coder) or CCS-P (Certified Coding Specialist-Physician) required.
• Experience with ICD codes required.
• Proficiency with Microsoft Office products (Word, Excel, Access, PowerPoint, Outlook).

Responsibilities

• Audit and abstract medical records to identify and submit ICD codes for CMS risk adjustment.
• Support coding decisions using standard evidence and tools.
• Ensure codes are accurate and supported by documentation.
• Identify active vs historical conditions and ensure clinical relevance.
• Adhere to timelines and project deadlines.
• Conduct self-audit to ensure internal policy compliance.
