Summary:
Responsible for electronically completing the functions of imaging and/or importing medical record documents, as well as performing manual operational duties to ensure the timely and accurate availability of patient information in accordance with government and accrediting regulations.
Responsibilities:

- Complies with department and hospital policies and procedures, including reviewing policies, procedures, and the Employee Handbook.
- Completes mandatory safety management education at hospital-wide, departmental, and job-specific levels.
- Participates in departmental and interdepartmental quality improvement activities.
- Locates and retrieves medical records from Health Information Services and nursing units for scheduled or unscheduled requests.
- Coordinates medical record copying and preparation for patient transfers, documenting information in the Release of Information system, and completing patient transfer sheets.
- Locates medical record numbers or patient names in the Patient Information Computer System.
- Maintains and verifies medical record numbers and names in computerized and manual systems.
- Processes healthcare records, examining documents, adding barcode stickers, and inserting DIP pages as needed.
- Prints Patient Index Pages and prepares records for electronic document management.
- Prepares records and loose documents for scanning, ensuring quality standards are met, and re-scanning if necessary.
- Follows up on missing records to ensure timely receipt and processing.
- Sorts loose documents for scanning and maintains accuracy and productivity standards.
- Reports discrepancies to the Health Information Systems Manager.
- Utilizes the Medical Record Tracking System for various record management tasks.
- Provides providers with deficient information and content requirements.
- Understands policies and regulations related to birth and fetal death certificates, minors, and affidavits of paternity.
- Reports system or user concerns promptly.
- Answers department phone promptly for customer service.
- Checks for medical record requests in SoftMed and dictated reports in eScription.
- Processes records requiring State and Federal reporting, including cases of child abuse, neglect, and Workers' Compensation injuries.
- Prepares and reports productivity statistics accurately and timely.
- Performs other duties as directed.

Qualifications:
- High school diploma.
- Ability to operate a computer terminal for data entry and retrieval.
- Limited knowledge of medical terminology.
Physical Requirements:
- Sit for 3-7 hours, stand, walk, and squat/kneel for 1-3 hours daily.
- Lift 10-35 lbs with proper body mechanics.
- Use both hands/wrists with moderate deviation from neutral position and perform gross motor skills.
- Vision: accurate near/far vision, color discrimination, depth perception.
- Hearing: able to hear whisper at 3 feet.
