Job description

The Care Facilitator is often the first contact a patient has with our ChenMed family. This role involves serving as a patient advocate and delivering exceptional customer service. Responsibilities include checking patients in and out of the clinic, providing basic secretarial and administrative support, and building trusting relationships with patients.

ESSENTIAL JOB DUTIES / RESPONSIBILITIES :
  1. Greet patients, record information, check-in and check-out patients, and schedule appointments. Maintain lobby organization and cleanliness.
  2. Manage schedules for PCP and specialists, ensuring top 40 patients are scheduled every 13 days.
  3. Use computer systems to add patient information, print forms, respond to emails, and prepare New Patient packets. Ensure contact information is accurate.
  4. Make 'love' calls, send birthday and greeting cards for special occasions.
  5. Answer phones, take detailed messages, and direct messages appropriately.
  6. Communicate information about clinical personnel to patients and others.
  7. Verify insurance, collect copays, and assist with scheduling follow-ups and referrals.
  8. Escort patients to exam rooms, ensuring their comfort and well-being.
  9. Support HEDIS initiatives by monitoring scheduled patients and reporting hospitalizations and census data.
  10. Present customer service surveys during check-out and address issues promptly.
  11. Participate in daily and weekly team huddles to discuss patient care.
  12. Coordinate transportation services, including managing driver lists, deliveries, and ride scheduling.
Other responsibilities may include :
  • Maintaining confidentiality of patient information.
  • Reviewing patient data for accuracy.
  • Transmitting correspondence via mail, email, or fax.
  • Performing additional duties as assigned by management.
KNOWLEDGE, SKILLS AND ABILITIES :
  • Exceptional customer service skills and a passion for serving others.
  • Knowledge of medical insurance and electronic medical records (EMR).
  • Understanding of ChenMed products, services, policies, and procedures.
  • Proficiency with phones, computers, software, and basic IT systems.
  • Professional and courteous communication skills.
  • Attention to detail for accurate reporting and data entry.
  • Strong verbal and written communication skills.
  • Excellent listening skills.
  • Positive, professional attitude.
  • Proficiency in Microsoft Office Suite (Excel, Word, Outlook).
  • Willingness to travel up to 10% regionally.
  • Fluency in English, both spoken and written.
EDUCATION AND EXPERIENCE CRITERIA :
  • High school diploma or equivalent required.
  • Graduation from a Medical Assistant program preferred.
  • At least 1 year of experience in a medical clinic or similar environment required.
  • Hourly pay range: $19.00 - $21.00.
  • BLS certification for Healthcare Providers preferred.
