Job summary

The position requires a Bachelor’s degree (preferred) and experience in case management, with licenses encouraged where applicable. A valid driver's license and reliable vehicle are required.

Qualifications

• Bachelor's degree preferred in related field.
• Licensed Social Worker preferred.
• Two years of case management experience preferred (or three years in a healthcare setting without a bachelor's).

Responsibilities

• Provide case management services to assigned clients.
• Address barriers to health and social needs for improved outcomes.
• Work with patient caseloads and undertake medically and socially challenging issues.
