Job summary

Qualifications

• Associate’s degree in human service field required.
• Bachelor’s degree in related field preferred.
• Experience or interest working with individuals living with mental illness, substance use disorders, and/or psychosocial stress.
• Bilingual fluency in Spanish/English or other critical language preferred.

Responsibilities

• Provides clinical interventions such as needs assessment, resource navigation, skill teaching, and care coordination to adult clients in community support program.
• Engages and motivates clients to participate in treatment services.
• Provides most services in community settings and related travel.
• Completes detailed documentation in the EMR and updates treatment plans.
• Assists with discharge planning and coordination with other providers.
