Job summary

TriHealth is seeking a Medical Assistant/Medical Home Coordinator in West Chester Township, OH to provide direct patient care in a primary care setting and coordinate care delivery. Qualifications

• Graduate of an approved healthcare program; clinical experience preferred.
• Equivalent experience accepted in lieu of a degree.
• Must be certified or registered medical assistant.
• Must maintain certification/registration as RMA, CAN, or CPR.
• Knowledge of medical office flow and front-office tasks.
• Ability to make quick decisions with thoughtful consequences.
• Knowledge of EMR, practice management software, and medical coding/billing encouraged.

Responsibilities

• Room patients for adult and pediatric visits with procedures and immunizations per protocol.
• Document rooming data, calls, orders, prescriptions, and portal messages; escalate when needed.
• Maintain clinical competency after orientation and annually.
• Support population health and close care gaps via pre-visit planning and daily huddles.
• Understand preventive schedules and communicate care gaps to physicians.
• Participate in ongoing education and training opportunities.
• Coordinate with wrap‑around services and community resources to support chronic disease management.
