Job summary

Telos Health Systems in Memphis, TN is seeking a Medical Director & Skilled Nursing Facility Primary Care Physician (MD/DO) to provide on-site primary care and lead medical operations in SNFs, supported by CMD training.

This role offers uncapped RVU-based earnings, a separate Medical Director stipend, a sign-on/retention bonus up to $20,000, and strong NP/PA backing, with telehealth and no overhead burden.

Qualifications

MD or DO with active Tennessee license.
Board certified or eligible in Internal Medicine, Family Medicine, Geriatrics or Emergency Medicine.
Experience or interest in post-acute care and leadership.

Responsibilities

Provide on-site primary care to skilled nursing facility residents.
Evaluate residents for significant changes in condition.
Collaborate with interdisciplinary care teams and facility leadership.
Supervise NP/PA clinicians providing routine and regulatory care.
