Job summary

Telos Health Systems, Inc. seeks a physician (MD/DO) to provide on-site primary care in skilled nursing facilities and to serve as Medical Director. The role includes fully funded CMD training and a separate Medical Director stipend, with no overhead burden and strong NP/PA support.

The position offers a flexible, part-time schedule, regular telehealth options, and a pathway into clinical leadership within post-acute and long-term care medicine.

Qualifications

• MD or DO degree required.
• Board Certified or Board Eligible in Internal Medicine, Family Medicine, Geriatrics, or Emergency Medicine.
• Active Kentucky medical license (or ability to obtain).
• Interest in post-acute care, leadership development, and community-based medicine.

Responsibilities

• Provide on-site primary care services to residents in skilled nursing facilities.
• Assess and manage acute and chronic medical conditions.
• Evaluate residents experiencing significant changes in condition.
• Collaborate with interdisciplinary care teams and facility leadership.
• Supervise and support NP/PA clinicians delivering routine and regulatory visits.
• Participate in quality improvement and patient care initiatives.
