Job summary

Telos Health Systems is seeking a Physician (MD/DO) to serve in a combined role providing on-site skilled nursing facility primary care while functioning as Medical Director, supported through CMD training. This position offers uncapped RVU-based compensation with a fixed MD stipend and strong NP/PA support, plus comprehensive benefits.

Responsibilities include supervising clinicians, collaborating with facility leadership, and participating in a defined after-hours call schedule with telehealth

Qualifications

MD or DO degree required.
Board certified or board eligible in Internal Medicine, Family Medicine, Geriatrics, or Emergency Medicine.
Active Tennessee medical license or ability to obtain.

Responsibilities

Provide on-site primary care services to skilled nursing facility residents.
Collaborate with interdisciplinary care teams and facility leadership.
Supervise NP/PA clinicians providing routine and regulatory care.
Participate in structured after-hours call schedule and telehealth on-call coverage.
Oversee and support care plans within the SNF setting.
Ensure regulatory compliance and quality improvement initiatives is supported.
