Job Description

Seeking a dynamic physician leader to expand their existing Palliative Care program and oversee the development and implementation of a new CMS Seriously Ill Population program.

Responsibilities include:

1. Growing the Palliative Care program with strong support from administration, hospital medicine team, specialists, advanced practice providers, and the clinical team.
2. Collaborating with an interdisciplinary team to establish and implement care plans for inpatient and outpatient services.
3. Supervising and mentoring Palliative Care Nurse Practitioners.
4. Innovating an existing transitional care program in partnership with an area rescue ministry.

Qualifications:

• Board Certified
• Fellowship training is preferred but not required with relevant experience.
