Job Description

The Medical Coordinator is responsible for coordinating medical and psychiatric services for adults with developmental disabilities and supporting them in achieving personal health outcomes.

Primary Responsibilities
  • Ensure the rights, health, safety and personal outcomes of residents are supported at all times.
  • Coordinate and schedule all medical and psychiatric services, including routine, follow‑up, and emergency care.
  • Confirm appointment locations, arrange transportation, and provide directions, ensuring timely arrival.
  • Gather all required information prior to appointments (consultation forms, medical book, nurse’s notes, etc.).
  • Collect completed consultation forms and medications from physicians before leaving for appointments.
  • Listen attentively to medical professionals, ask clarifying questions, and avoid inappropriate interruptions.
  • Review the medical appointment calendar daily to view scheduled appointments.
  • Document treatments accurately and communicate information to all relevant team members.
  • Maintain medication inventory and place orders as needed.
  • Set up and manage paper or electronic filing systems/databases, updating documentation and maintaining records.
  • Operate electronic systems and coordinate the flow of information both internally and with external organizations.
  • Ensure all required documentation is completed, processed in a timely manner, and meets quality and compliance thresholds.
  • Perform other duties as assigned.

Qualifications

Work Environment

The role is performed in-person with travel between locations and doctors’ offices.

Physical Requirements
  • Sit, walk, and stand for prolonged periods (at least 8 hours).
  • Communicate verbally and in writing.
  • Operate standard office equipment such as computers, phones, photocopiers, fax machines, and filing cabinets.
  • Reach with hands and arms; bend, stoop, kneel, crouch, or stand on a stool as needed.
  • Lift up to 50 pounds at a time.
