Job summary

Covenant Medical Group is seeking a Coder Analyst to analyze documentation in patient records, assign ICD-10-CM and CPT-4 codes, and support accurate billing. The role involves data abstraction, assisting the Billing Department, and ensuring coding compliance across encounters.

Responsibilities include reviewing records, verifying data for charge entry, and collaborating with physicians to ensure proper coding and timely reimbursement. Experience preferred but not required for all candidates.

Qualifications

• Experience coding ICD-10-CM and CPT-4 required to ensure proper sequencing for billing and reimbursement.
• One year of coding experience for physician services or completion of a recognized billing and coding course.
• Professional coding experience is preferred.

Responsibilities

• Review documentation in the medical record to determine ICD-10 CM and CPT-4 coding needed to comply with billing guidelines.
• Verify data in the medical record and abstract pertinent information for charge entry.
• Use CPT-4 and ICD-10 standards to assist providers with code selection and assignment.
• Edit unbilled claim transmission reports daily and correct as needed for timely billing.
• Participate in quality coding and audit reviews for each provider.
• Assist providers with coding questions for services rendered.
• Refer coding questions to the Operations Manager when further research is needed.
• Contact physicians for clarification and medical necessity.
• Review encounters for accurate documentation and coding of services rendered.
• Communicate with office manager and CDI supervisor regarding pending items.
• Support adherence to practice quality and quantity standards and participate in quality improvement.
• Performs other duties as assigned.
