The role requires a bachelor's degree and at least two years in human services; proficiency with Microsoft Office and strong interpersonal skills are essential. A compassionate, organized professional is favored.

Qualifikationen

• Bachelor’s degree in psychology, counseling, mental health, human services or related field or equivalent combination of education/experience.
• Two years of work or volunteer experience in human services or related activities.
• Licenses & Certifications: Mandatory Reporter Training; Chauffeur’s license if transporting clients; both within intro period if not current.

Aufgaben

• Completes intakes and ongoing assessment to identify needs, goals and access resources.
• Assesses eligibility for financial and medical benefits and educates on entitlements.
• Collaborates with medical professionals to support the health plan, including adherence and harm reduction.
• Monitors and documents client contacts, needs, progress and significant occurrences per grant guidelines.
• Coordinates with PHC services and external referrals for comprehensive care (medical, dental, mental health, housing, etc.).
• Conducts case consultations for clients who have fallen out of care and takes steps to re-engage.
• Advocates for clients with community agencies, schools and other organizations.
• Ensures transportation access for appointments following PHC policy.
• Communicates with agencies using proper releases of information.
• Conducts outreach at community sites and serves as PHC liaison.
