Job summary

ACLD in Kings Park, NY, is seeking a Medical Coordination Specialist to provide medical coordination and direct support to people with intellectual and developmental disabilities in a residential setting.

You will collaborate with the residential nurse, coordinate appointments, manage medications, and document care plans to promote health, safety, and independence for residents in alignment with Life Plans.

Qualifications

• Must communicate effectively in English, both written and verbal.
• AMAP, SCIP Core, CPR, First Aid and AED certifications required or obtainable.
• One year experience with developmentally disabled population preferred.

Responsibilities

• Oversee dispensing and documentation of medication per AMAP procedures and guidelines.
• Maintain stock of medications for residents, including OTC products.
• Coordinate weekly nursing activities with the residential nurse.
• Schedule and accompany medical appointments, collect and submit paperwork.
• Prepare MARs monthly and ensure accuracy of related records.
• Assist residents with daily living tasks and transportation as needed.
• Ensure program environments meet ACLD quality standards and regulatory requirements.
• Support families and document care plans and communications.
