Job summary

ACLD is seeking a Medical Coordination Specialist to support people with intellectual and developmental disabilities at our Ronkonkoma residential site. You will coordinate medical care, advocate for residents, and help maintain health and safety through hands-on guidance and life-skill coaching.

This role requires English communication, AMAP/SCIP CPR First Aid when needed, and the ability to document clearly and work with a nurse and care team.

Qualifications

• High School Diploma or equivalent required.
• Ability to communicate and understand in English; both verbally and in writing.
• The ability to obtain (within 90 days of hire) and maintain AMAP Certification.
• The ability to obtain (within 180 days of hire) SCIP Core, CPR, First Aid and AED Certifications.
• One year experience with developmentally disabled population preferred.
• The ability to assist with lifting and transferring a person up to 15 lbs.
• The ability to perform SCIP maneuvers which may involve bending, kneeling, crouching and lifting.

Responsibilities

• Oversee the dispensing and documentation of medication in accordance with AMAP procedures.
• Maintain appropriate stock of medications for each person supported.
• Meet with the residential nurse to conduct weekly nursing coordination.
• Schedule all medical appointment follow ups as directed.
