Job summary

Habitat Health is seeking a Medical Assistant to support PACE center operations and participant care. You will assist with intake, vital signs, and routine clinical tasks in collaboration with the IDT.

The role requires completing an accredited MA program, current certification, phlebotomy credentials, and strong communication skills to ensure safe, timely, and coordinated care for older adults.

Qualifications

• High school diploma or equivalent and completion of an accredited Medical Assistant program.
• Current Medical Assistant certification or credential as required by state law.
• Current phlebotomy certification and ability to perform routine clinical procedures.

Responsibilities

• Complete intake, rooming participants, obtain vital signs and screenings.
• Perform lab testing, phlebotomy, EKGs, immunizations and related procedures.
• Communicate observations and changes in status to support coordinated care.
• Coordinate follow-up visits and external appointments with the care team.
• Serve as first point of contact for participant questions and care needs.
• Maintain accurate intake and clinical documentation in the EMR.
• Support clinical operations, referrals, and authorization processes.
• Maintain clinic readiness including logs, exam rooms, equipment and inventory.
