Group Health Cooperative of South Central Wisconsin. BETTER TOGETHER

0.90 FTE / 36 hours per week.

This position is eligible for a $3000 INCENTIVE for joining GHC!
(Amount prorated based on FTE)

The Care Team Specialist (CTS) participates as a member of a care team and supports high-quality patient care, including engaging patients in maintaining their physical and mental well-being. Responsibilities include pre-visit preparation, monitoring patient flow, rooming patients, obtaining patient histories and vital signs, managing test results per protocols, supporting Epic in-basket workflow, returning phone calls, and assisting with medical examinations and procedures. The CTS collaborates with Registered Nurses and Medical Providers in patient care management. They review team/provider in-baskets and My Chart messages for their care team. Practitioners and RNs may delegate tasks. Candidates must have completed medical assistant training, LPN training, Paramedic, or EMT training from an accredited program.

Job Specific Minimum Qualifications

  • Must meet requirements for a Medical Assistant (MA), Licensed Practical Nurse (LPN), or EMT.
  • Medical Assistants must have completed an accredited MA training program and be eligible for or hold national certification within six months, or have three years of experience with current certification from recognized organizations (CCMA, AAMA, ARMA, NCCT).
  • LPNs must have completed an accredited LPN program and hold current licensure in Wisconsin or obtain it within six months.
  • EMTs must have completed an approved training program and hold a valid Wisconsin license (EMT, AEMT, or Paramedic). CPR/AED certification is required within four months of employment.
  • Ability to travel to various sites, with personal transportation and a valid Wisconsin driver license.
  • Desirable: One year of outpatient clinical experience.
  • Knowledge of clinical procedures, equipment, and supplies; computer applications including EMR systems (experience with EPIC is desirable); medical terminology; basic pharmacology; and administrative procedures.
  • Strong communication, interpersonal, and cultural sensitivity skills; ability to maintain confidentiality; physical ability to perform duties including lifting up to 50 pounds and standing for prolonged periods.
