The role emphasizes accurate vitals, phlebotomy, immunizations (where certified), EHR documentation, care coordination, and health coaching to improve population health outcomes.

Qualifications

• High School diploma or GED required; Bachelor's in Health Science preferred.
• Brief MA competencies within one year of hire.
• Basic familiarity with computers and web-based apps.

Responsibilities

• Develops empathetic relationships with patients, families and colleagues to support patient care.
• Facilitates patient flow and communicates delays to patients and providers.
• Transmits hand-offs between patient and provider with clear information.
• Performs MA duties: taking vitals, height/weight, EKGs, POC testing, phlebotomy, immunizations (certified MA).
• Leads conversations for population health goals through outreach and health coaching.
• Maintains equipment, ensures sterility, and manages inventory.
