Travel to facilities up to 60 miles from your home may be required. This position typically operates on a Monday-Friday dayshift schedule.

* Mileage reimbursement plan is offered.

Provider Support

  • Assist with any day-of changes due to acute needs, hospitalizations, admissions, or discharges.
  • Prepare clinical documentation for assigned providers daily.
  • Complete basic opening of encounter notes as per discipline protocol.
  • Perform medication reconciliation on all scheduled patients at each encounter.
  • Enter appropriate facility chart data into patient encounter notes and assign to provider.
  • Enter lab and diagnostic study results into patient encounters and assign to providers as per guidelines.
  • Assist with collecting and sorting all clinical paperwork needed for clinician review, ensuring completion each day.
  • Host telehealth sessions for providers as assigned, including routine and acute visits.
Facility Support
  • When scheduling is part of your role: Share patient schedules with key facility staff as needed.
  • Assist with triage needs as directed by the assigned provider, whether in or out of the building.
  • Assist with triage documentation as needed.
  • Ensure all necessary paperwork is routed appropriately for the facility, Eventus providers, office, or families.
  • Assist with obtaining consents for services as needed.
  • Attend stand-up meetings to report facility needs and relay information between facility and Eventus providers.
  • Assist with discharge planning of clinical services.
  • Check in and out daily with facility staff regarding patient status and visits.
  • Maintain patient confidentiality at all times.
  • Document encounters and services appropriately.
  • Check communication channels several times daily to meet patient needs.
