Job description

This care is provided to adults who reside in skilled nursing, assisted living facilities, and private residences through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Psychologists, Licensed Clinical Social Workers, and Clinical and Administrative Support Staff.

The Medical Assistant is responsible for enhancing the performance of the clinical care team by supporting and facilitating clinical services with their assigned providers. The Medical Assistant is required to work in the field with the assigned provider as directed by the Lead Medical Assistant or MA Manager/Supervisor. You may be asked to travel to facilities up to 60 miles from your home. This position will typically be a Monday-Friday dayshift.

* Mileage reimbursement plan is offered.

Provider Support
  • Ensure that the clinical provider in the assigned building is able to focus on taking care of their patients for the day.
  • Ensure that each patient is seen as medically necessary per clinical protocol.
  • Serve as a “buffer” between provider and families/staff to facilitate proper patient management.
  • Document appropriate encounters and time for CCM and BHI services.
  • If scheduling is assigned: Update scheduling rosters daily to ensure accuracy and timely schedule communication.
  • Assist with any day-of changes to scheduled visits due to acute needs, hospitalizations, admissions, or discharges.
  • Prepare clinical documentation for assigned providers each day.
  • Complete basic opening of encounter notes as per discipline protocol.
  • Perform medication reconciliation on all scheduled patients during each encounter.
  • Enter appropriate facility chart data on all scheduled patients into patient encounter notes and assign to provider.
  • Enter lab and other diagnostic study results into patient encounters and assign to providers per guidelines.
  • Assist with collection and sorting of all clinical paperwork needed for clinician review and ensure completion each day.
  • Host telehealth sessions for providers as assigned by manager/supervisor, including routine and acute visits.
Facility Support
  • When scheduling is part of your role: Share patient schedules with key facility staff as needed.
  • Assist with triage needs as directed by assigned provider, whether they are in or out of the building that day.
  • Assist with triage documentation as needed.
  • Ensure all necessary paperwork is routed for the facility to Eventus providers, office, or families.
  • Assist with obtaining consents for needed services.
  • Attend stand-up meetings to report relevant information between the facility and Eventus providers.
  • Assist with discharge planning of clinical services as needed.
  • Check in and out daily with facility staff regarding patients seen and their status.
  • Maintain patient confidentiality at all times.
  • Document appropriate encounters and time for CCM and BHI services.
  • Check all communications several times daily to meet patient needs.
  • Inform patients and families of recent visits and new orders, deferring to providers when needed.
  • Communicate transitions of care to the CCM team to improve patient care and education.
Additional Duties and Responsibilities
  • Adhere to Eventus WholeHealth standards, policies, procedures, and employee handbook, including HIPAA rules and regulations.
  • Follow the dress code policy: clean, wrinkle-free, matching scrubs, appropriate closed-toed footwear, and wearing a name badge.
  • Report to your Lead Medical Assistant for any additional assignments when duties are completed early.
  • Seek further training if unsure how to proceed with any task.
Qualifications/Education
  • High School Diploma or equivalent.
  • Valid driver’s license and reliable transportation.
  • Expected travel up to 1 hour from home address.
  • Typing speed of 45 wpm or greater.
  • Medical scribing experience preferred.
