Job description

Eventus WholeHealth delivers an integrated model that provides holistic care for medically vulnerable adults, achieving extraordinary outcomes for those we serve. This care is provided to adults residing in skilled nursing, assisted living facilities, and private residences through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Psychologists, Licensed Clinical Social Workers, and support staff.

The Medical Assistant is responsible for enhancing the performance of the clinical care team by supporting and facilitating clinical services with their assigned providers. The Medical Assistant is required to work in the field with the assigned provider as directed by the Lead Medical Assistant or MA Manager/Supervisor. You may be asked to travel to facilities up to 60 miles from your home. This position typically operates on a Monday-Friday dayshift schedule.

Provider Support
  1. Ensure that the clinical provider in the assigned building is able to focus on patient care for the day.
  2. Ensure that each patient is seen as medically necessary per clinical protocol.
  3. Serve as a “buffer” between provider and families/staff to facilitate proper patient management.
  4. Document appropriate encounters and time for CCM and BHI services.
  5. If scheduling is assigned: Update scheduling rosters daily for accuracy and send schedules timely.
  6. Assist with any day-of changes to scheduled visits due to acute needs, hospitalizations, admissions, or discharges.
  7. Prepare clinical documentation for assigned providers each day.
  8. Complete basic opening of encounter notes as per discipline protocol.
  9. Perform medication reconciliation on all scheduled patients at each encounter.
  10. Enter appropriate facility chart data into patient encounter notes and assign to provider.
  11. Enter lab and diagnostic study results into patient encounters and assign as per guidelines.
  12. Assist with collection and sorting of clinical paperwork needed for review, ensuring completion daily.
  13. Host telehealth sessions for providers as assigned, including routine and acute visits.
Facility Support
  1. When scheduling is part of your role: Share patient schedules with key facility staff as needed.
  2. Assist with triage needs as directed by the assigned provider, whether in or out of the building.
  3. Assist with triage documentation as needed.
  4. Ensure all paperwork is routed for the facility to Eventus providers, office, or families.
  5. Assist with obtaining consents for services needed.
  6. Attend stand-up meetings to report information between the facility and Eventus providers.
  7. Assist with discharge planning of clinical services as needed.
  8. Check in and out daily with facility staff regarding patient status.
  9. Maintain patient confidentiality at all times.
  10. Document encounters and services appropriately.
  11. Check communication channels multiple times daily to meet patient needs.
  12. Inform patients and families of recent visits, new orders, and defer to providers when necessary.
  13. Communicate care transitions to the CCM team to improve patient care and education.
Additional Duties and Responsibilities
  • Adhere to Eventus WholeHealth standards, policies, procedures, and employee handbook, including HIPAA rules.
  • Follow dress code policy: clean, wrinkle-free, matching scrubs, closed-toed footwear, and name badge.
  • Report to your Lead Medical Assistant if work is completed early for additional tasks.
  • Seek further training if uncertain about any duties.
Qualifications/Education

High School Diploma or equivalent

Valid driver’s license and reliable transportation

Expected travel up to 1 hour from home

Typing speed of 45 wpm or greater

Medical scribing experience preferred
