Under the direction of the department manager, the Medical Assistant will be assisting the team in referral management for High-Risk patients and helping to assist with patient needs.

Receive new patient referrals from clients
Create charts for all new patients
Input medical information in new chart
Help Care Coordinators with patient needs as applicable
Foster relationships with their team to ensure the CareHarmony culture is positive and supportive
