Job summary

The New York Foundling seeks a MAP Coordinator to support the coordination of medical care for persons with developmental disabilities. You will ensure timely appointments, arrange transportation, advocate for healthcare rights, and maintain accurate health records in a compassionate, person-centered environment.

This role requires strong organizational and communication skills, the ability to navigate complex systems, and collaboration with clinicians, caregivers, and community resources.

Qualifications

• High school diploma or GED is required; healthcare-related certification or degree is a plus.
• Valid driver’s license with up to 6 points and state residency where applicable.
• Minimum 1 year in healthcare coordination, patient advocacy, or related field.
• Strong organizational, communication, and interpersonal skills; familiarity with developmental disabilities.

Responsibilities

• Schedule and coordinate medical appointments and tests, with reminders and follow-ups.
• Arrange transportation to appointments; accompany as needed to facilitate communication.
• Advocate for healthcare rights and access for persons with developmental disabilities.
• Maintain accurate records of visits, diagnoses, medications, and care plans; participate in teams.
• Provide information on procedures and medications to promote health literacy and self-management.
• Respond to emergencies during appointments and coordinate follow-up care.
• Collaborate with healthcare professionals, caregivers, and community resources for continuity of care.
• Participate in corrective action plans and audits as needed.
