Job summary

CareSource in Mobile, AL is seeking a Team Lead for Integrated Care Coordination - Waiver. This role combines hands-on supervision of Care Coordinators with managing a reduced caseload of MyCare HCBS Waiver members, ensuring timely assessments and service plans.

The Team Lead serves as the first escalation point for clinical and operational issues, coaches staff, and supports adherence to Waiver requirements, regulatory standards, and member outcomes.

Qualifications

• Associates degree in healthcare required.
• Three years of care management/HCBS Waiver experience.
• Leadership experience preferred.
• RN or LSW licensure preferred or required where applicable.

Responsibilities

• Provide day-to-day oversight and direction to assigned Care Coordinators.
• Monitor workflows and ensure timely completion of assessments, service plans, contacts, and outreach.
• Serve as first escalations point for staff questions and operational issues.
• Coach, mentor, and develop assigned staff with real-time feedback.
• Support compliance with MyCare HCBS Waiver requirements and regulatory standards.
• Assist in onboarding and staff development and facilitate interdisciplinary meetings.
