Job summary

Health & Human Services Commission (HHSC) is seeking a Program Support Case Manager to join the Program Enrollment and Support unit in Grand Prairie. You will facilitate STAR Kids and HCBS eligibility, coordinating with MCOs and internal HHSC departments to ensure timely enrollment and service delivery.

The role requires strong knowledge of Medicaid waiver programs, policy interpretation, and excellent communication. Regular schedule and telework options may apply per policy.

Qualifications

• High School Diploma or GED required.
• Prefer experience with case management.
• Prefer experience interpreting complex policy.
• Strong written and verbal communication skills.

Responsibilities

• Attends work on a regular and predictable schedule.
• Develop, coordinate, and explain waiver programs to applicants and members; authorize or deny services.
• Register waiver program eligibility in systems and coordinate enrollment with MEPD.
• Assist members transitioning from nursing facilities to the community.
• Handle requests and maintain documentation for fair hearings and appeals.
