Job summary

LifePath Systems seeks an Enrollment Specialist in Texas to guide individuals through Medicaid Waiver enrollment, assess needs, and connect them with essential services. You will collaborate with families, the treatment team, and external agencies to promote independence and community participation.

The role requires a trauma-informed, person-centered approach, strong communication skills, and the ability to de-escalate crises while maintaining professionalism and cultural sensitivity.

Qualifications

• Bachelor's degree or higher preferred for enrollment work.
• Associate degree in social/behavioral/health-related field plus one year experience acceptable.
• High school diploma with two years relevant experience acceptable.

Responsibilities

• Complete enrollment into Medicaid Waiver programs.
• Identify needs, develop plans, and connect individuals with resources.
• Communicate effectively with families, individuals served, and stakeholders.
• De-escalate potential crisis situations and maintain professional demeanor.
• Coordinate care with LifePath Systems team and external agencies.
