This is an opportunity for a proactive, resourceful professional who enjoys solving problems, building relationships, and taking ownership of daily operations while helping expand access to home care throughout Maine.

What You’ll Do
MaineCare Client & Caregiver Coordination
  • Coordinate Medicaid‑funded personal care and personal support services within the agency’s approved programs.
  • Manage caregiver schedules, staffing assignments, and service coverage.
  • Assist with caregiver recruitment, interviews, onboarding, and retention.
  • Coordinate new member referrals, admissions, and timely service initiation following required approvals.
  • Maintain communication with members, families, caregivers, and service coordination teams.
  • Respond promptly to staffing challenges, missed visits, and service interruptions.
MaineCare Program Coordination
  • Coordinate agency services in accordance with applicable MaineCare Section 19 and Section 96 requirements.
  • Review service authorizations, approved hours, and applicable service plans.
  • Communicate with Service Coordination Agencies (SCAs), service coordinators, and referral partners.
  • Monitor service utilization and address potential authorization or service interruptions.
  • Maintain accurate documentation and follow up on referrals and ongoing service needs.
Operations & Compliance
  • Monitor Electronic Visit Verification (EVV), visit documentation, and scheduling accuracy.
  • Identify and help resolve documentation discrepancies and service‑related concerns.
  • Assist with incident reporting, quality assurance, and operational follow‑up.
  • Track unstaffed hours, caregiver availability, and outstanding service needs.
  • Support MaineCare provider requirements and company policies.
  • Use home care management software to maintain accurate records and coordinate with supervisory or clinical personnel as needed.
Program Growth & Community Relationships
  • Build positive relationships with SCAs, MaineCare referral sources, and community partners.
  • Support member referrals, caregiver recruitment, and service‑area growth.
  • Identify opportunities to strengthen service delivery and expand access to home care.
Requirements
Required Qualifications
  • At least 1 year of direct, hands‑on experience with Medicaid‑funded home care services, Medicaid waiver programs, or Home and Community‑Based Services (HCBS).
  • Experience with service coordination, caregiver scheduling, staffing, or client service management in a Medicaid‑funded home care environment.
  • Familiarity with service authorizations, documentation, and Medicaid program requirements.
  • Strong organizational, communication, problem‑solving, and follow‑through skills.
  • Ability to work independently and manage competing priorities.
  • Proficiency with computers, electronic documentation, and scheduling systems.
Highly Preferred Qualifications
  • 2 or more years of direct Medicaid home care or waiver coordination experience.
  • Direct experience with MaineCare Section 19 or Section 96.
  • Knowledge of MaineCare personal support or personal care services.
  • Experience with Maine Service Coordination Agencies and Medicaid referral sources.
  • Familiarity with EVV, including Sandata or comparable systems.
  • Experience with AxisCare or similar home care management software.
  • Caregiver recruitment, onboarding, retention, or Medicaid program growth experience.
Who Will Succeed in This Role?

Someone who takes ownership, anticipates staffing challenges, communicates clearly, follows through on commitments, maintains accurate documentation, and builds strong relationships with members, caregivers, and referral partners.

Compensation
