Rocky Mountain Health Care is seeking an Eligibility Advocate in Colorado Springs to establish and maintain Medicaid eligibility for program participants. You will coordinate with DHS, SEP, and other agencies to ensure timely Medicaid approvals and accurate documentation, acting as the primary point of contact for eligibility and community resources.

The role requires knowledge of Medicaid/Medicare requirements, strong organization, and experience with EHR and CBMS.

Formación

• Associate’s degree or equivalent in a human services environment, business, or related experience.
• Thorough understanding of DHS, Medicaid, VA, and Social Security programs.
• Certified Medicaid Professional I (MCMP-I) is highly desired.
• Knowledge of community resources for older adults and their families.
• Prior case management experience is favorable.
• Strong time management, organization, and attention to detail.

Responsabilidades

• Initiate Medicaid, SNAP, and Long-Term Care Medicaid Waiver applications for participants.
• Track application status to ensure timely enrollment and census.
• Review CBMS and contact DHS to obtain eligibility status for participants.
• Serve as primary contact for eligibility-related inquiries and documentation.
• Coordinate records requests from doctors, banks, insurers, and other agencies.
• Assist with establishing income trusts and coordinating with partner agencies.
