Job summary

VNS Health in New York seeks a Community Liaison to help prospective members and families access Medicaid, MLTC, and community resources. You will perform outreach, assess eligibility, and coordinate enrollment with the care team.

This role offers a comprehensive benefits package, opportunities for growth, and travel between regions as needed. An associate degree and health/human services experience are preferred.

Qualifications

• Minimum of two years' work experience in a health care or human services setting.
• Knowledge of Medicaid eligibility and Medicaid Managed Care regulations (preferred).
• Bilingual skills may be required, as determined by operational needs.

Responsibilities

• Performs outreach to community agencies to establish relationships and refer services.
• Assists clients to obtain Medicaid and connects them to MLTC programs.
• Screens financial records to assess enrollment eligibility.
• Documents services and maintains records per policy.
• Participates in clinical team meetings and coordinates with plans staff.
• Arranges transportation and appointments for members.
