Job summary

CVS Health Corporation seeks an Engagement Outreach Hub Care Management Associate to coordinate telephonic outreach and enrollment in care management programs. You will blend sales, navigation, and customer support to engage members and connect them to care management services.

The role emphasizes compliance, documentation, and responsive problem solving, with a schedule Monday–Friday 8am–5pm Central Time in a full-time capacity.
