Job summary

Ponos Management, Inc. is expanding access to healthcare and social services through an Eligibility Specialist role at Ponos Care in Richmond, VA.

You will help members navigate Medicaid and state benefit programs, coordinate resources, and act as a single point of contact for eligibility determinations across the care continuum. Qualifications

• High school diploma or equivalent; some college preferred.
• Experience in VA health/human services benefits applications.
• Excellent communication and organizational abilities.

Responsibilities

• Liaise with members to ensure access to care and social assessments; address social determinants of health.
• Develop case management plans using organization and community resources.
• Educate and communicate with members, families, and healthcare providers.
• Initiate referrals for social services and assist with eligibility determinations.
• Coordinate with the Department of Social Services and other stakeholders for timely services.
• Advocate for patients and families to ensure needs are understood by the care team.
• Document thoroughly in member health records as required.
