Ponos Management, Inc. is seeking an Eligibility Specialist to support access to healthcare and social services for Medicaid populations.

You will interview members, coordinate eligibility determinations, and connect them with resources to address social determinants of health. In this role you will collaborate with members, families, providers, and community organizations, manage referrals, and document processes to ensure timely, equitable access to benefits and services nationwide.

Qualifikationen

• High school diploma or equivalent; some college preferred.
• Experience in a call center or healthcare customer service handling benefits apps.
• Strong experience with Illinois Medicaid, ABD, TANF, SSI programs.
• Excellent verbal and written communication skills.
• Organizational and multitasking abilities; proficient with EHR systems.

Aufgaben

• Liaise with members to ensure access to care and provide home and social assessments.
• Develop case management plans using organization and community resources.
• Collaborate with members, families, and healthcare providers.
• Initiate referrals for social services and assist with eligibility processes.
• Document health information accurately in the member's records.
• Advocate for patients and coordinate LTSS/SSI applications with state programs.
