Job summary

Sunrise Senior Living is seeking a Wellness Nurse LPN to monitor resident health, oversee medications, and maintain clinical quality within the community.

You will perform assessments, document care, and collaborate with families, physicians, and the Resident Care Director to support resident well-being and regulatory compliance.

Qualifications

• Active LPN license in good standing.
• Graduate of approved nursing program.
• 1+ year experience in senior care or clinical setting.
• Strong assessment, documentation, and communication skills.

Responsibilities

• Conduct move-in, 3-day, semi-annual, and change-of-condition assessments.
• Perform monthly wellness visits and monitor health trends.
• Follow up on falls, incidents, and hospital returns.
• Transcribe physician orders and ensure accurate MAR/TAR documentation.
• Complete medication audits and oversee medication compliance.
• Partner with families, physicians, and the Resident Care Director.
• Support infection control and regulatory standards.
• Educate and guide care staff on documentation and medication practices.

What We’re Looking For:

Pay Range:

29.15 - 38.90
