Job summary

Lake Seminole Square in Lewis Center, OH is seeking a compassionate LPN/Medication Technician for a full-time day shift in memory care. You'll administer medications, monitor residents, document care, and support care plans in a resident-centered environment.

Qualifications

• Ohio Licensed Practical Nurse (LPN) license in good standing is required.
• Experience in Assisted Living, Memory Care or geriatric nursing preferred.

Responsibilities

We are seeking a compassionate, dependable, and service-focused LPN/Medication Technician to join our Assisted Living and Memory Care team on Full-Time Day Shift. This position plays a vital role in promoting resident health, safety, comfort, and quality of life while providing exceptional care in a warm, resident-centered environment.

Schedule

• Full-Time
• Day Shift
• Weekend and holiday rotation as needed

Responsibilities

• Administer medications and treatments in accordance with physician orders and community policies.
• Monitor residents for changes in condition and promptly communicate concerns to the healthcare team.
• Complete accurate and timely documentation of medication administration, treatments, and resident care.
• Assist with resident assessments, service plan updates, and care coordination.
• Provide support and guidance to caregivers regarding resident care needs.
• Ensure compliance with state regulations and community standards.
• Respond appropriately to emergencies and resident incidents.
• Foster positive relationships with residents, families, providers, and team members.

Qualifications

For LPN Candidates:
