Job summary

HMR Veterans Services seeks a Registered Nurse to join the Colonel Robert L. Howard State Veterans Home in Pell City, AL as an MDS Coordinator.

This on-site, full-time role coordinates resident assessments (MDS) in line with federal/state regulations and facility goals, and develops individualized care plans. The position requires a nursing degree and an active Alabama RN license, with at least one year of healthcare experience.

Qualifications

• Active RN license in Alabama required.
• Nursing degree (BSN or higher) required.
• Experience in hospital or long-term care settings preferred.
• Strong knowledge of nursing practices and resident care standards.

Responsibilities

• Coordinate timely and accurate MDS assessments per regulations.
• Develop and update individualized care plans based on assessments.
• Ensure 14-day admission MDS and 21-day care plans timelines.
• Conduct quarterly, annual, and significant change assessments.
• Collaborate with Interdisciplinary Care Plan Team and involve families as appropriate.
