Job summary

Serenity Estates of Morris is seeking a compassionate MDS RN or LPN to join our dedicated healthcare team in Morris, IL. You will conduct MDS assessments, develop individualized care plans in collaboration with the interdisciplinary care team, and ensure therapies and medications are administered safely to residents.

Qualifications

• Current RN or LPN license in the state of Illinois.
• Minimum of 1-3 years of experience in senior care or a related field.
• Strong understanding of MDS documentation and assessment protocols.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct MDS assessments and develop individualized care plans in collaboration with the interdisciplinary care team
• Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive care
• Provide direct care to residents, including medication administration and mobility assistance
• Monitor and report changes in resident condition to the care team
• Participate in quality improvement initiatives to enhance resident outcomes and satisfaction
• Maintain accurate and detailed records of resident care and assessments
